SUMMARY:
- Grade: £32,154 (NJC 19)
- Hours: Full-Time (37 h.p.w.): Fixed term: to 30th March 2027 (pending future funding)
- 25 days holiday per year plus bank holidays, 8% pension contribution (1% employee contribution), mileage rate, cycle to work scheme.

OVERVIEW:
The Community Connector will be based at Langworthy Cornerstone and employed by the Charity and will work towards the Partnership objectives of the Wellbeing Matters Programme.
The Wellbeing Matters programme is a Voluntary, Community and Social Enterprise led initiative delivered by Salford CVS that focuses on Person and Community Centred Approaches to improving the wellbeing of Salford people. Wellbeing Matters is led by Salford CVS in partnership with five VSCE anchor organisations which employ a Community Connector/s (The Big Life Group, START inspiring Minds, Social Adventures, Inspiring Communities Together and Langworthy Cornerstone).
A key element of Wellbeing Matters is the community connecting / social prescribing programme that connects people referred by health professionals with a non-medical need to local community assets to support their wellbeing, independence, and to reduce social isolation. The programme aims to support people to feel more connected to their communities, more confident, and less socially isolated by linking them into local groups and services. The goal is for people to be better equipped to manage their own health and wellbeing.

Our Live Well Community Connector (Adult Social care) Champion for older adults will act as a vital link, connecting individuals and their families and carers to local resources, support services, and opportunities that enhance wellbeing, reduce isolation and loneliness in older adults, and promote independence. This role is key to fostering a sense of belonging and helping adults remain active and engaged within their community with an aim to reduce barriers to accessing support and services.
The Community Connector will take a person-centred approach, providing a safe and supportive space for individuals to discuss their goals, needs, and aspirations. To increase confidence and help individuals better manage their own wellbeing. A key advocate for the age friendly Salford offers and continue Salford’s legacy being a WHO age friendly city. They will share information about relevant events and activities, facilitate peer connections with others who share similar experiences, and offer practical support to improve access, inclusion, and overall quality of life for older people.
Our community connector in this space would hold experience in supporting older adults and carers and families. Experience and understanding aging well are fundamental to the role. A responsibility and commitment for staying up to date on support for older adults is required.
The role will use a community development and a social model of health approach to effectively build a proactive, collaborative approach embedded within the community to support older adults to age well.
Main Tasks and Responsibilities
- Build relationships. Develop positive working relationships and accept referrals from the adult social care’s multi-disciplinary team in this city-wide post.
- Provide Information and have local knowledge. Share details about local initiatives, activities, and resources that are accessible and supportive of older adults in Salford, connecting to key networks (Salford older persons network, Age Friendly Salford, Carers network, older adult’s mental health teams), health and wellbeing teams and VCSE networks. Participate in Team Meetings, share information, and upskill Adult social care team on available resources.
- Offer Practical Support. Attending groups and services with older adults to assist in finding support groups and accessing services that can help manage daily challenges and other practical advice and support offers on financial, social and creative activities.
- Integrated working. Working across key city partners to ensure older adults get a coordinated response across health care, adult social care and the VCSE sector.
- To manage a caseload of clients. Developing and maintaining effective working relationships with key relevant community-based service providers to ensure effective and smooth onward referrals e.g. other local voluntary, community and social enterprise groups and organisations.
- One team approach. Liaise with the wider Wellbeing Matters’ team of neighbourhood Community Connectors and Volunteering Development Workers to ensure that clients can access local provision, working under the direction of the Social Prescribing Operations Manager.
- Raise awareness of specialist support. Provide insight, information and support to the wider Wellbeing Matters team on older adult support. Actively contribute to the mapping of services and activities available to support clients in the community, identify gaps in provision and feed information into Wellbeing Matters delivery team meetings and to the Wellbeing Matters neighbourhood Volunteering Development Workers

- To commit to your own personal development and attend training, team meetings or development activities as required
- To undertake any other duties as appropriate to the nature and grading of the post
